Cagrilintide is a long-acting amylin analogue: the 37-residue amylin backbone with proline substitutions and a C20 fatty diacid on its first lysine. This listing is a preloaded pen with 3 mL of ready-made solution in bacteriostatic water, 20 mg of cagrilintide in all, so there is no powder to reconstitute. The research on cagrilintide concerns amylin receptor pharmacology and clinical trials of the peptide alone and with semaglutide.

What is in the pen

Each pen contains a 3 mL glass cartridge of sterile aqueous solution. Cagrilintide is present at 0.67% w/v, 20 mg per pen, which is about 6.7 mg/mL, or roughly 1.5 µmol/mL at the theoretical mass of 4409.0 Da. The solvent is bacteriostatic water with 0.9% benzyl alcohol as an antimicrobial preservative. On the safety data sheet, cagrilintide is classed Repr. 2 (H361, suspected of damaging fertility or the unborn child), and benzyl alcohol is listed as harmful if swallowed or inhaled, a serious eye irritant and a possible skin sensitiser.

Keep the pen at 2 to 8 °C in the original carton, out of the light, capped between uses and locked away from unauthorised persons. Never freeze it, and discard it if the solution has frozen, discoloured or become cloudy. Freeze-dried cagrilintide is sold in Cagrilintide 10mg vials, and peptide pen vs vial explains what changes between the formats.

What the research covers

Amylin is a 37-amino-acid hormone from pancreatic beta cells, and its receptors are multi-subunit G protein-coupled receptors built from a core receptor protein and receptor activity-modifying proteins1. Its main site of action, according to Hay and colleagues, is the circumventricular organs of the central nervous system1. Human amylin readily forms amyloid fibrils, which made it a difficult drug template, and the earlier analogue pramlintide, used with insulin in diabetes, has a short half-life2. Kruse and colleagues described the structure-activity work behind cagrilintide, which they chose for clinical development in obesity because it was stable and long-acting2.

Human studies began with cagrilintide alone: a 26-week phase 2 trial compared five cagrilintide strengths with liraglutide and placebo, with the percentage change in body weight as the primary endpoint3. It enrolled adults without diabetes and masked participants and investigators to active versus placebo treatment3. Later trials combined cagrilintide with semaglutide. None of them used a pen like this one or any research solution, and the abstracts give no data on how long cagrilintide stays intact in bacteriostatic water, so the storage limits on the safety data sheet are the ones to follow.

The full literature, from the receptor potency data to the phase 3 programme and its safety tables, is in the cagrilintide and CagriSema guide.

Frequently asked questions

How does the pen differ from the cagrilintide vials?

The vials hold freeze-dried peptide kept at −20 °C and dissolved in the lab at whatever concentration the work needs. The pen holds 20 mg already dissolved in 3 mL of bacteriostatic water and is kept at 2 to 8 °C, never frozen.

What concentration is the solution?

0.67% w/v, which is 20 mg in 3 mL or about 6.7 mg/mL. At cagrilintide's theoretical mass of 4409.0 Da that is roughly 1.5 µmol/mL, or about 4.5 µmol in the whole pen.
